Transforming and redefining adult and social care with advanced digital solutions.
The Integrated Digital Care Solution is revolutionising adult and social care services through biometric sensors and data-driven insights. Funded by the £600,000 5G Glasgow City Region (GCR) Smart and Connected Social Places Innovation Fund, this pioneering initiative enhances care delivery, promotes independent living, and optimises resource management for local authorities across Scotland.
The Challenge
Scotland’s ageing population and the rising prevalence of chronic health conditions are putting increasing pressure on local authorities to deliver effective adult and social care services. Traditional frailty assessments, often reliant on visual observations, lack the precision needed for proactive care. This results in delayed interventions, increased hospital admissions, and strained healthcare resources.
The Innovation
The Integrated Digital Care Solution is transforming this landscape by harnessing the power of biometric sensors and advanced data analytics. By continuously monitoring vital signs and aggregating data into an intelligent platform, care providers can gain real-time insights that support timely, targeted interventions. This proactive approach improves patient outcomes while enabling more efficient use of healthcare resources.

Bringing this vision to life requires strong partnerships. Mari-One Ltd leads the project, overseeing data integration and analytics. They work closely with the North Lanarkshire Adult Social Care Partnership, which provides real-world testing environments. Medi Bio Sense Ltd supplies state-of-the-art biometric sensors, while Amba Health Ltd ensures secure data infrastructure. Adding academic rigor, the University of Strathclyde contributes its expertise in AI and predictive analytics, helping to refine and advance the solution.
Since its implementation, the project has already shown significant results. Participants report feeling more confident managing their health, knowing that early signs of frailty can be detected and addressed promptly. Care teams find the system invaluable for improving decision-making, allowing them to focus on those who need the most support. The reduction in hospital admissions speaks volumes about the effectiveness of early interventions driven by real-time data.
Local authorities have embraced the system’s ability to optimise resource allocation, ensuring that care is not only effective but also efficient. The integration of customisable dashboards has streamlined workflows, enhancing both operational efficiency and user engagement.
