Smplicare’s AgeWell Coaching Platform is transforming adult care and wellbeing. Funded by the £600,000 5G Glasgow City Region (GCR) Smart and Connected Social Places Innovation Fund, this innovative digital intervention supports older adults by reducing isolation, enhancing health, and fostering connected care.

The Challenge

Falls are a leading cause of injury among older adults, with 30% of those over 65 and 50% of individuals over 80 experiencing a fall annually. This not only impacts physical health but also leads to social isolation, increasing the risk of depression, cognitive decline, and chronic illnesses.

Falls cost the UK over £4.4 billion annually, with each sever fall potentially costing the NHS up to £46,000. By preventing falls and reducing isolation, Smplicare not only improves lives but also eases pressure on healthcare services.

The Innovation

The Smplicare project involves distributing Fitbit Inspire 3 fitness trackers to participants aged 55 and over who have experienced a fall in the past year, and integrating data from the wearable to provide a personalised coaching and falls prevention exercise programme through the Smplicare AgeWell Coaching App.

The app, powered by the Smplicare Fall Risk Algorithm (SFRa) and AI-driven coaching, delivers real-time health insights, personalised recommendations, and connected care through wearable devices. The platform promotes fall prevention, encourages physical activity, and reduces isolation by fostering meaningful social connections.
